Astitva Ek Prem Kahani takes a leap of 18 years. Manas s son Siddhant Awasthi known as Sonu grows up to be a doctor and is working with Simran for her Astha general hospital. Neha s baby Gudiya who is adopted by Simran and is named as Astha also aspires to be a doctor. Astha becomes a spoiled brat because of Simran s excessive pamper. Siddhant keeps scolding Astha for her reckless behavior. Astha hits a lady with her bike but is unaware that the lady is Siddhant s mother Urmila.
